CLASS REPORTING - WEEKLY

Please complete the form below for each class you teach. This is crucial to program operations, and we cannot complete your payment if we have not received your submissions. (Even if you are donating your time and not claiming a stipend, please fill out this form, so we can properly account for the populations we are serving, clearly communicate with our program partners, and more.)

  • [NEW] IMPORTANT: If you are submitting a form for a class taught at Blaine St. or Rountree, please also include a photo of the class roster in the last file upload section. (Milestone tracking can be found here.)

INVOICING - MONTHLY

  • The invoice template can be found here.

  • You can double check class rates in our scheduling spreadsheet

  • Please save as a PDF and upload with one of your monthly class submissions below. 

    • If this is your first time submitting an invoice, please be sure to submit your W2 in the file upload option below or by emailing to katie@yogaforallmovement.org.

    • While invoices are due the 5th of the month, it’s ideal to complete your class data forms right after each class you teach, so that it is fresh in your mind.
